Immunizations (vaccines) are available to children and adults. For more information, please view the Immunization Clinic brochure.

Notice: Parents/legal guardians unable to accompany their child(ren) to their vaccination appointment, please follow the following directions.

Print and complete the Immunization-TST Consent Form for EACH child with an appointment. Send this form with the adult bringing your child(ren) to their appointment. If available, attach a recent copy of child(ren)'s vaccination record to the consent form. If you are unsure of what immunizations your child needs, please call our immunization nurse at 910-253-2330.
